Continuity of Care

Dr. Sanjiv K. Sharma is an internist with a specialization in geriatrics. Although at Geriatric Medical Center, we specialize in geriatric care for the elderly, we encourage and invite young adults (18 & older) into our care. That is where the continuity of care begins. Continuity of care is a continuous relationship sustained over time between patients and their care providers. We try to handle most of your medical conditions and health care in one place.

 

Continuity of care is even more important in the later stages of life as it unites the care between treatment, providers, and facilities.  There is less of confusion as the patient’s entire medical history is in once place. A sense of confidence and understanding is already built with the doctor, who oversees the patient’s care regardless of whether he/she is in a nursing home, assisted facility, rehabilitative facility, hospitalized, or at home. Dr. Sharma tends to his patients in a number of hospitals and nursing homes/rehabilitation facilities. Such vertically integrated coordination and care prevents errors and ensures appropriate follow-ups and transitions within the health care system. With the coordinated sharing of information, there is no need for the patient to repeat him/herself.

 

Patients under the continuity of care have shown improved adherence to prescribed screening and treatments. There has been lower use of the emergency rooms and fewer hospitalizations as well. Over all, the process is cost effective and time saving. More importantly, however, continuity of care establishes consistency to achieve the ideal medical care and enhance the quality of life for patients and their families.
